COMPANY BENEFITS PACKAGE

 

MEDICAL

 

Your choice of HMO or PPO health care insurance.  Employees are eligible on the first of the month following 30 days of continuous employment to elect coverage for a combination of medical, dental, and vision.

BIENNIAL (PERIODIC) MEDICAL EXAM

 

Cox pays D.O.T. physical renewals for employees.

DENTAL

 

Employees are eligible on the first of the month following 30 days of continuous employment.

VISION

 

Employees are eligible on the first of the month following 30 days of continuous employment.

GROUP LIFE INSURANCE

 

The first of the month following 30 days of continuous employment, employees receive $10,000 of life insurance, and up to $100,000 is available.

LONG TERM DISABILITY (LTD)

 

Long Term Disability is available to all employees following 30 days of continuous employment.

401(K) PLAN

 

Employees who are full time and have completed 90 days of employment are eligible to enroll.    The plan permits contributions limited only by those imposed by the IRS.  Employees are provided four open enrollment periods annually.  The company will also match your contributions, 25% on the dollar, up to the first $2,000 per year of employee contribution.

PAID HOLIDAYS

 

Thanksgiving, Christmas, New Years and your birthday are paid holidays.

VACATION PAY

 

1 week after the 1st year

2 weeks after the 2nd year

3 weeks after the 5th year

4 weeks after the 11th year

SAFETY PROGRAM INCENTIVE

 

The Safety Program Incentive is designed to award each driver equally, based on the driver's safe working and driving practices. Certain terninals have this incentive in place.
